The form is the same build in every practice area. The fields are not. These are the ones each configuration captures before a consultation is booked.

Personal Injury

Date and location of the incident, injuries reported, treatment already sought, the insurance carrier, and the opposing party for a conflict check.

Immigration

Current status, country of origin, any filing deadline or hearing date, and prior applications.

Family Law

Matter type, the other party for a conflict check, whether children are involved, county of residence, and any existing order.

Criminal Defense

The charge, the arresting county, custody status, bond if it has been set, and the next court date.

SOUTH FLORIDA

Nearly nine in ten Miami-Dade County residents age five and older speak a language other than English at home, 88.0%, per the U.S. Census Bureau’s QuickFacts. A form that only exists in English is not a minor gap here. It is most of the market. The county does not publish a Spanish-specific figure, but the large majority of that non-English population is Spanish-speaking, which is why English and Spanish, not just English, is where a South Florida intake form has to start.

And the Spanish has to be right. Machine translation reads as foreign in Miami, and to the daughter filling out the form for her father it says the firm did not care enough to finish.
